Who we are

At Welfare Connections Australia, we are dedicated to empowering individuals with disabilities to lead fulfilling, independent, and inclusive lives. Our team brings together over 8 years of combined experience in disability support, underpinned by professional training and a deep commitment to person-centered care.

Our founders, Patience Kabiche and Dr Chati Tasantab, have extensive backgrounds in providing complex support across diverse settings. We work with children, youth, and adults, and our experience spans community participation, supported independent living, and positive behaviour support. Our team members are trained to deliver tailored care—supporting personal goals, facilitating access to community activities, and promoting social inclusion.

We partner with leading organizations and adhere to best practice standards in delivering direct care, therapeutic and positive behaviour support, medication administration, and respectful management of restrictive practices. Our approach is holistic: we support daily living, skill development, and community engagement, always respecting the rights and aspirations of every individual we serve.

Our Commitment

Dedicated to a person-centred approach, we work in close partnership with clients and their families to develop flexible support plans that truly reflect each individual’s strengths, preferences, and aspirations. Through ongoing collaboration and open communication, we empower clients to take an active role in shaping their own supports, ensuring their voices are always heard and respected.
